A refund that was issued but never arrived can be traced, and this is the form that starts that process.
It is worth doing properly the first time. An incomplete submission produces a request for more information and another wait, which is frustrating when the point of filing was to stop waiting.
This covers when a payment is genuinely overdue rather than merely slow, what to gather before submitting, how the form is completed and sent, what happens afterwards, and what to do where a cheque was cashed by somebody else.
What Is Form 3911?
IRS Form 3911, Taxpayer Statement Regarding Refund, is the form you file to initiate a refund trace when your tax refund has not arrived, was lost, or was stolen. Filing Form 3911 triggers an official IRS investigation into what happened to your refund.
Gather everything before you submit: the return it relates to, the exact amount, the date it was issued if you know it, and the bank details given on the return. A complete submission is dealt with once. An incomplete one produces a request for more information and another wait.

What If I Changed Banks After Filing?
If your direct deposit was sent to a closed account, the bank must return the funds to the IRS. The IRS will then reissue the refund as a paper check to your address on file. This can add several weeks to the process.

Frequently Asked Questions
Can I file Form 3911 for a stimulus payment that was missing?
For prior-year Economic Impact Payments that were not received, the process varies by year. The IRS has issued guidance on claiming the Recovery Rebate Credit on the relevant year's tax return rather than using Form 3911 for those payments.
What if I never received my state refund?
Form 3911 is a federal IRS form only. For a missing state refund, contact your state's department of revenue directly.
I moved since filing. Will the check come to my new address?
Only if you updated your address with the IRS (via Form 8822) or USPS mail forwarding is active. File Form 8822 to change your address with the IRS proactively.
What should be gathered before making the enquiry?
The return the payment relates to, the exact amount expected, the date it was issued if you know it, and the bank details given on the return. If a direct deposit was returned by the bank, their correspondence helps. Having all of it to hand makes a single submission sufficient, where an incomplete one usually produces a request for more information and another wait.
How long should we wait before starting this?
Long enough that the payment is genuinely overdue rather than simply slow, and the waiting periods differ depending on how the payment was sent. Filing too early achieves nothing and can complicate matters if the original arrives while the enquiry is open. Check the current waiting periods in the form's own instructions, since they have changed.
